An investigation is under way following the death of a baby born in the Midland Regional Hospital in Mullingar, the Health Service Executive has confirmed.
The child was born on 16 August, but died in the neo-natal unit at the National Maternity Hospital in Dublin less than a week later.
In a statement, the HSE said its "incident management policy" had been implemented and a "full and thorough investigation" had begun into the circumstances surrounding the infant's death.
It said the child's family was involved in the process.
