The Health and Information Authority has moved to close down a private nursing home in Clara, Co Offaly.
The authority secured an interim court order to de-register the Upton House Private Nursing Home on the grounds that there was a risk to life, or a serious risk to the health or welfare of 13 residents there.
The HSE is now making alternative arrangements for the care of the patients.
Upton House has been operating as a nursing home in Clara since 1975.
The facility has been inspected on four occasions by HIQA.
Last night, Judge Gerard Haughton granted an interim order in Portlaoise District Court in cancelling its registration as a designated care centre.
The court was told 107 breaches of regulations have been identified and two serious allegations made with regard to the care of a 95-year-old woman who fractured bones in her legs after falling out of bed twice in the one hour and a 58-year-old man who was found incoherent and requiring critical medical care.
