The Health Service Executive has welcomed the report into the misdiagnosis of Rebecca O'Malley.

The HSE says that it has already implemented a number of the report's recommendation on clinical and process issues.

However, the HSE has expressed its disappointment that the report does not establish the reasons behind the five-month delay in the review of Ms O'Malley's cytopathology specimen, a process which can be carried out in two weeks.

A special phone line has been established for current or former patients of Mid-Western Regional Hospital Limerick or Cork University Hospital who may have concerns about their care.

The numbers are 061-483286 and 061-483287 for Limerick and 1850 742 000 for Cork.

The report into the delayed diagnosis of breast cancer in Ms O'Malley has heavily criticised the HSE and the two hospitals.

The Health Information and Quality Authority report says the missed diagnosis in March 2005 was a once-off interpretative error by a locum consultant pathologist at Cork University Hospital.

It says that this mistake in itself may not have led to a delayed diagnosis for Ms O'Malley, had a fully functioning multidisciplinary review meeting about the case been held at the Mid-Western Regional Hospital in Limerick.

The report says there are serious issues in the way the HSE and its two hospitals in Limerick and Cork responded to Ms O'Malley's concerns, as well as communication issues between the hospitals and the wider HSE.

It finds that there was a delayed and disjointed response to Ms O'Malley, which it says is a symptom of underdeveloped and ineffective management systems within the hospitals.

The report says the quality of cytology specimens sent by the Mid-Western Regional Hospital to Cork University Hospital was poor.

The report makes 15 recommendations to safeguard future care for patients.
