The investigation into the death of a 16-year-old girl at University Hospital Limerick last month will begin shortly and will be chaired by an external person.
Aoife Johnston, from Shannon in Co Clare, died from meningitis on Monday 19 December.
Speaking on RTÉ's News at One, Chief Clinical Director of UL Hospitals Group Professor Brian Lenehan said the review will involve the family and all those involved in Aoife's care.
The terms of reference put a three-month timeframe for conclusion of the review, he said.
Prof Lenehan said he wanted, as Chief Clinical Director, to extend his deepest condolences to the Johnston family "on the tragic loss of their daughter".
He added: "I'm the Chief Clinical Director, I'm ultimately responsible for patient safety, quality and patient safety, and it will be for me to commission that review, which will start very shortly.
"It will be chaired by someone who's external to our hospital group and the results would then be shared with the family, but I can't really speak to the specifics at this stage in respect to the Johnston family."
