A report into RTÉ's Toy Show The Musical has found there is no written record of the RTÉ Board ever granting its approval for the musical that amassed losses of €2.2 million.
The probe by Grant Thornton is set to be published tomorrow morning.
It has identified a lack of governance overseeing the venture.
It will say that there was an obligation on the RTÉ Board to give clearance for the venture that was staged in the Convention Centre in Dublin in 2022.
However, the board was never formally asked for a decision to proceed with Toy Show The Musical, the report found.
The Grant Thornton Report found that there was no formal consideration by the RTÉ Board until after a contract was signed with the venue for the musical.
The procedures for previous large projects at RTÉ running to more than €2 million were not followed in this case.
It also found that the level of sponsorship may have been overstated.
And there is a suggestion that the show would have struggled to break even.
In a statement, the RTÉ Board said the report will be published "as soon as possible".
"The Grant Thornton report into Toy Show The Musical, commissioned by the Audit and Risk Committee (ARC) of the RTÉ Board, is now complete," it said.
It added: "The report has been furnished to the ARC and the RTÉ Board, and also subsequently to the minister. The report will be published as soon as possible."
TD calls for new RTÉ committee appearance
Minister for Media Catherine Martin said she has received the report and is giving it due consideration.
The minister said she will not be commenting on the report until RTÉ publishes it, which she expects will happen in the coming days.
Taoiseach Leo Varadkar is understood to have told his Fine Gael TDs and Senators tonight that sacking the board of RTÉ, on foot of the latest revelations about Toy Show the Musical, is not a good idea.
Two sources at the the parliamentary party meeting said the Fine Gael leader suggested such a move might be the "easy" option, but having no board at such a time is not good as - once gone - the members are unaccountable.
Fine Gael TD Brendan Griffin has called for RTÉ Director General Kevin Bakhurst and chair of the board Siún Ní Raghallaigh to be "hauled" before the Oireachtas Media Committee again.
It is understood the Kerry deputy told the Fine Gael parliamentary party tonight that RTÉ had failed to cooperate with the committee and information it had sought was still outstanding.
Mr Griffin, a member of the committee, is believed to have claimed that the only time RTÉ provides information is in advance of a committee appearance.

Documentation given to the Oireachtas Committee on Media last July confirmed losses for the musical reached €2.2 million, with just over 11,000 tickets being sold for the shows.
The documents detailed the individual costings such as venue hire and production set-up costs of €658,500, while the running costs were €622,016.
The average ticket price was quoted at €46.50. This meant RTÉ needed to sell 70% across all shows at the 1,995-seater venue to break even.
Just 20,000 people went to see the show. Only 11,044 of them were paying customers, with 5,500 tickets supplied to guests on a complimentary basis and a further 3,500 awarded as prizes.
In reality, there were just 27 live shows. The total venue capacity for the shows that went ahead was 53,865.
The accounts also detail a forecast of €156,020 for marketing and press but the eventual expenditure was €339,634 - an overspend of €183,614.
