The Health Service Executive has apologised unreservedly for the "deficits in care", which it said culminated in the untimely and tragic death of 25-year-old Maxine Maguire in February 2017.
The HSE told the High Court that it wanted to offer her parents, Kathy and Robbie Maguire, from Newbridge in Co Kildare, and the wider family its heartfelt condolences for the devastating loss and the continuing profound sadness and suffering for the family.
Ms Maguire had suffered mental health issues for some years and had received treatment.
In December 2016 she presented to Naas Hospital emergency department and was later sent home.
Ms Maguire spent a lengthy period in another hospital after a suicide attempt and underwent a liver transplant there before being discharged.
She died on 7 February 2017 at St James's Hospital in Dublin following another suicide attempt.

Speaking after the apology, Kathy Maguire said the family had taken the case to hold the HSE accountable and for justice for their beautiful daughter.
She said it had not been an easy journey and had taken a huge toll on the entire family.
Ms Maguire had secured a degree in theology and sociology from Maynooth University, and was pursing child, youth and family studies at Carlow IT. She enjoyed music festivals and travelling.
The 2019 inquest into her death recommended that a psychiatric consultant should have a face-to-face review with a patient before the decision to discharge is made.
The inquest said standard operating procedures should be put in place to make it easier to re-enter the mental health services after an extended period of discharge.
It also recommended that all medical files should be on a computer system accessible to all relevant medical professionals.
