The Health and Safety Authority says an investigation into the death of a three-year-old boy in a lift in Galway last year is nearing completion.
Solomon Soremekun died after falling into a lift shaft at the Hynes building on St Augustine Street, on 13 January 2014.
He sustained serious injuries in the incident and was pronounced dead at the scene.
It is understood the investigation has focused on how the boy managed to fall into the lift shaft of the three storey building.
HSA officials have conducted a detailed technical examination of the building in question over a period of several months last year.
The lift in which the accident occurred was removed for further technical analysis.
It is expected that a file will be sent to the Director of Public Prosecutions, upon completion of the investigation.
A number of issues relating the use of lifts in public buildings have been identified as a result of the inquiry.
The HSA says it plans to intensify a national inspection campaign to ensure lifts are operating to the correct standards in the coming months.
Solomon Soremekun's mother, Omolara Alabi intends to take legal action against the owners of the building following the incident.
The family had only been in Galway a number of weeks when the death occurred.
