On Tuesday, the Minister for Justice Jim O'Callaghan delivered an apology to Shane O'Farrell’s family.
It came almost 14 years after he was killed in a hit-and-run by a driver who should have been in jail at the time.
The minister said that the justice system left the 23-year-old exposed to a threat that "he should not have been exposed to".
The driver of the car was Zigimantis Gridzuiska, who was a repeat offender on bail.
A court ruled that he should not have been out at the time, and Mr O’Callaghan said there were a series of failures that led to this not being the case.
