Inquest opens into death of two-day-old baby

An inquest has begun in Dublin into the death of a baby boy a few days after his birth at the Midland Regional Hospital Mullingar in 2015.

Baby Stevie Cullivan was born on 28 April, 2015 and died two days later, after his transfer to the Rotunda Hospital in Dublin.

Sandra Cullivan, from Longwood in Co Meath, told the inquest she learned she was pregnant in August 2014.

A few months later in December, her husband died suddenly of a heart attack. She said she was devastated but kept going with her baby due.

Ms Cullivan said she was booked into Mullingar hospital on 28 April 2015 to give birth and was due to be induced.

She was in a private room and was given pain relief just after 10pm. She was then left alone in a bed for about 90 minutes.

Ms Cullivan said she felt a lot of pressure at 11.30pm and was very concerned. She could not reach the call bell and shouted for help.

A nurse came and said she would have to be brought to the labour ward quickly. Ms Cullivan described the scene in theatre as utter chaos.

She said baby Stevie was not crying when he was born at 11.59pm and it was obvious he was not okay.

She was told he would have to be transferred within around four hours to the Rotunda.

Ms Cullivan said baby Stevie did not reach the Rotunda until around 8.30am to 9am. He died there just before 1.30pm.

Ms Cullivan told the inquest she had put her faith in Mullingar hospital and they had left her in labour for so long without monitoring.

Lawyers for Mullingar hospital told the inquest that the HSE had apologised in writing to Ms Cullivan.

An internal review was conducted that identified shortcomings and recommendations were made.

The inquest is before Dublin District Coroner Dr Myra Cullinane and is scheduled to last two days.

Carolyn McCrory, staff midwife at Mullingar, told the inquest that she did an examination on Ms Cullivan at around 10pm on the evening of 28 April.

She said Ms Cullivan was not in labour, as the contractions were short and not frequent.

Concepta Lyster, a midwife at Mullingar hospital, said she had not heard Ms Cullivan call for help.

She said when she saw her at 11.30pm Ms Cullivan was quite distressed.

On examination, she found her nearly ready to deliver, so the plan was to get her to the labour ward as soon as possible.

The inquest has been adjourned until tomorrow.
