An inspection of University Hospital Limerick has found failures in relation to medication safety.

The announced inspection by the Health Information and Quality Authority in May found a lack of effective systems to ensure minimum standards of safety and quality were met regarding medication safety.

There was also under-reporting of medication errors.

In response, the hospital provided a quality improvement plan, however HIQA said it had no timelines in it.

A follow-up inspection will take place in around six months.
