HSE Chief Executive Bernard Gloster is to step down from the role next March, he has announced.
Mr Gloster was appointed to the role in December 2022 and formally started in March 2023.
In a statement, he said that he will step down from the role and "retire from the public service on March 5th 2026".
Mr Gloster, who is 59, said he will continue to "serve in this very important role and for the coming months".
He said he wants to "bring certainty to the future leadership of the organisation" and as a result has submitted his resignation to the HSE Chair Ciaran Devane and Minister for Health Jennifer Carroll MacNeill.
This will allow "time to prepare for the next phase of leadership of our health and personal social services", he added.
Before being appointed to the role, where he replaced Paul Reid, Mr Gloster had served as CEO of Tusla.
Ms Carroll MacNeill said Mr Gloster had been an "extraordinary public servant" who had dedicated himself to "improving the health service for everyone in Ireland".
"Since he took over his position in 2023, Bernard has brought over 30 years of public sector experience to advance leadership across the HSE organisation and I have greatly appreciated his work, his expertise and his friendship in my short time as Minister."
"I look forward to working with him in the coming months to continue our good work to advance a health system that works better for all."
