An investigation is under way into the circumstances in which a woman was found dead in a disabled toilet at Galway University Hospital during the August Bank Holiday weekend.
A statement on behalf of the hospital said the toilet was locked in a public area adjacent to the hospital's emergency department.
The Saolta Hospital Group, which includes the hospital and is part of the Health Service Executive, said the woman entered the hospital building unknown to any member of staff.
She was not registered as a patient, either in the hospital or in the emergency department.
The case has been referred to the Galway West coroner.
The Connacht Tribune newspaper, which first reported the case, said the woman was undiscovered for up to 10 hours.
