The HSE's final audit report into the charity Console strongly criticises the organisation for hampering and delaying the investigation because of a lack of records and cooperation.
The report, which runs to more than 200 pages, noted that management's delays in answering questions and providing information made the audit of the suicide bereavement charity more difficult.
Much of the detail on the lavish spending by former chief executive Paul Kelly, his wife Patricia and son Tim Kelly had already emerged through the RTÉ Investigations Unit.
The Interim CEO of Console earlier said that there significant and imminent challenges facing the charity.
David Hall said his personal view is that the organisation might not survive, but added that the services must be protected.
He has called the 12 staff who work full time for the charity to a meeting tomorrow to update them.
Mr Hall said the Revenue Commissioners launched an investigation into the suicide charity and appears to be owed at least €70,000.
This is the sixth inquiry into financial irregularities at the organisation.
Speaking on RTÉ’s Today with Sean O'Rourke, David Hall said that the €70,000 figure was what was known to be owed.
He said Minister for Health Simon Harris and junior minister Helen McEntee need to make a call on whether the service should continue.
He said no donations were coming in and there was a cost in providing the service.
Mr Hall said protecting services should be paramount along with the team, but not necessarily the organisation.
The Health Service Executive gives around €70,000 a month to the charity but the cost is around €100,000 a month.
Mr Hall said a lot of money seemed to be used in direct debit and credit card transactions and there were legacy issues in terms of bills outstanding, including a €19,000 telephone bill.
Meanwhile, the HSE will attend the Public Accounts Committee on Friday 15 July to answer questions about Console and also the issue of top-ups in Section 38 agencies including St John of God.
The PAC has written to the HSE requesting copies of the internal audit of Console and the service level agreement.
