The Health Service Executive says assessments are being carried out to determine care plans for residents at the Áras Attracta centre in Co Mayo.
The HSE has denied there are plans to close the facility in Swinford.
However, it appears to be planning for a reduced capacity there, saying it expects many residents will choose to live in the community instead of at the centre.
In a statement, the HSE said it was consulting with families and residents at Áras Attracta, as part of an "assessment of need" process.
It says the nature of services provided there will change as care provision moves towards a community setting.
Last August, the Health Information and Quality Authority issued notice of a proposal to cancel the registration of one of the HSE run centres in Swinford.
This followed two unannounced inspections which raised concerns about care at Centre 2.
The issues related to management, governance and staffing.
The centre in question houses five Bungalows - 3, 4, 5, 6, and 13.
An undercover report by the RTÉ Investigations Unit in December 2014 showed some residents at a unit in Áras Attracta being slapped, physically restrained and shouted at.
