HIQA has determined after an inspection of a nursing home in Connemara that it had failed in its requirements in five areas of its operation.
It said the nursing home falls short with regard to its staffing levels, health and social care standards, medication and complaints management.
The inspection was carried out on Áras Chois Fharraige in An Spidéal in Co Galway.
The initial report said that the centre's governance and management require an immediate improvement to ensure the safety and well-being of all its 41 residents.
Last month HIQA carried out additional checks and discovered that the home had not carried out the remedial actions or that any improvement had not been sustained.
The first inspection report of the two published today on the home was primarily based on a one-day review at the end of June following tip-offs to HIQA.
Its inspectors' activities included meeting residents, relatives and staff.
The 41 residents were roughly equally divided between maximum, high and medium-dependency, with just four ranking as low dependency.
HIQA found that "suitable arrangements were not in place to cover the absence of the person in charge who was on leave at the time of the inspection".
It found "sufficient staff with the appropriate skill mix were not consistently rostered on duty to ensure that the assessed needs of residents were met in line with their choice".
The findings indicated "that all residents' health and social care needs were not met and some improvement to the recording of information about medication was required".
Inspectors said the recording and investigation of complaints had not been suitably recorded and investigated as required by statutory regulations.
They noted that an investigation initiated by the provider into allegations of abuse and misconduct was in progress.
The report found that the investigation was nearing completion and that the allegations had not been upheld.
In what is termed a "minor breach", HIQA found that one of two Complaints Registers failed to record details of an investigation, its outcome and the complainant's level of satisfaction with the outcome as required by the statutory regulations.
