A review group set up by the Health Service Executive has criticised the failure of health authorities to establish an independent investigation into allegations regarding patient care and safety at St Mary's Hospital in the Phoenix Park.
The group examined allegations covering the years 1996 to 2006 and concluded that they were of a serious nature and required a response of the utmost probity from health authorities at the time.
It said there were real and justifiable concerns regarding the health and safety of patients and staff, fire safety and hygiene matters and that these concerns were supported by written documentation.
The inquiry report has now become available, following a decision by the Information Commissioner that it be released in the public interest.
Meanwhile, the HSE said current standards at St Mary's have been independently assessed as high and it is investing in improving the physical environment of the 18th century building.
