Conan moves from flanker to number eight as regular skipper Caelan Doris starts on the bench, as do Dan Sheehan, Andrew Porter and Robbie Henshaw, the latter of whom will make his 100th appearance for the province.
Tadhg Furlong makes just his fourth start of the season at tighthead prop, coming in for Rabah Slimani, while Rónan Kelleher returns at hooker and Cian Healy comes in at loosehead.
The second row pairing of RG Snyman and Joe McCarthy is retained from the 62-0 win against Harlequins last week, with James Ryan still sidelined by a calf injury.
Max Deegan has been rewarded for his good form with a start at blindside flanker, as Josh van der Flier continues on the open side.
Jamison Gibson-Park and Sam Prendergast go again in the half-backs, while Jordie Barrett comes in for Robbie Henshaw to partner Garry Ringrose in midfield.
Tommy O'Brien gets a start on the right wing as Jamie Osborne misses out, with James Lowe and Hugo Keenan completing the side.
