Pablo Canavosio will start at scrum-half for Italy in Saturday's Test against Australia in Padua.
The Viadana player, who has 20 Italy caps mainly on the wing, has earned a starting spot at number nine having been the spare scrum-half on the bench during June's 26-0 defeat in South Africa and the 13-12 win in Argentina.
Among other position changes, Andrea Masi and Andrea Marcato swap their usual roles. Masi moves to full-back having played at fly-half during this year's Six Nations, when Italy beat Scotland but still finished bottom.
Marcato, full-back in the 23-20 win over Scotland, goes to fly-half while Mirco Bergamasco is due to start on the wing having traditionally been a centre.
Coach Nick Mallett has also named uncapped scrum-half Giulio Toniolatti as a replacement for the first of three November Tests, with the Azzurri also facing Argentina in Turin on 15 November and the Pacific Islands in Reggio Emilia on 22 November.
Australia, who will later meet England, France, Wales and the Barbarians, are due to name their team on Thursday.
