The Health Information and Quality Authority has published inspection reports on medication safety at the Midland Regional Hospital, Tullamore, the Rotunda Hospital in Dublin and St Michael's Hospital in Dun Laoghaire.
In general, the health watchdog found good safety medication practices in all three hospitals.
But it also found opportunities for learning and improvement in the hospitals.
The announced inspection at the Rotunda on 22 March last found scope for improvement by the hospital in the level of near-miss reporting.
At the Midland Regional, Tullamore on 13 March, HIQA found that most medication incident reports were submitted by clinical pharmacists.
It says the hospital needs to increase participation in the medication safety programme beyond the pharmacy department.
During the inspection of St Michael’s on 29 March, HIQA found that medication-related incidents were likely under-reported there and cited the need for improved clinical leadership in this area.
