Glen Campbell has been charged with drink driving and assault after an hit and run incident.
The country singer was arrested in his hometown of Phoenix, Arizona, on 25 November.
Campbell has been formally charged with aggravated assault, drunken driving, extreme drunken driving and leaving the scene of an accident.
Extreme drunken driving under Arizona law applies to a blood alcohol level of more than 0.15%.
Campbell, 67, apologised for his arrest last week and blamed it on the anti-anxiety drug Lexapro.
