Top scorer Stephen Lambe (0-14) was Monaghan's savour as he brought them back in both normal and extra-time, before hitting the match-winning point in the NHL Division 4 final at Kingspan Breffni Park.
Lambe forced extra-time with a last gasp puck with the score at 3-13 to 1-19 at the end of normal time. Michael Greaney scored the Monaghan goal.
Greaney's first half goal ensured a 1-14 to 2-07 interval lead but Down staged a late rally scoring 1-01 in injury-time before Lambe's late strike.
