The Inspector of Mental Health Services has been notified of an incident at St Stephen's Psychiatric Hospital in Cork.
An elderly patient was found dead in the hospital on Monday.
The Mental Health Commission said that the inspector is in contact with the Health Service Executive and the hospital, and has requested a full report on the incident.
Gardaí are also investigating the death.
