The Garda Síochána Ombudsman Commission is investigating the circumstances surrounding a fatal road traffic accident which occurred yesterday in Co Galway.
An off-duty garda, who was driving his own car, was involved in a collision with a transit van, which resulted in the death of a 65-year-old woman.
The woman, who was a passenger in the garda's car, was later named as Margaret O'Donoghue from Bandon, Co Cork.
