Italian investigators have concluded that the inexperience and ineptitude of American troops led to the killing of an Italian intelligence agent in Iraq.
Nicola Calipari was shot dead by US soldiers as he drove through a roadblock in Baghdad in March. He had been taking a freed hostage to safety.
Mr Calipari was hailed as a hero by Italians after his death. An American inquiry into the shooting had cleared the soldiers of blame.
The Italian government report also says the US authorities should have signalled that there was a checkpoint on the road.
But it adds that it was difficult to pinpoint individual responsibility for Mr Calipari's shooting.
